Planning the Later Chapters of Life with Jackie Melinger.

Jackie Melinger, co-chair of our HAZAK Committee and an Illinois Licensed Nursing Home Administrator, will discuss the importance of making sure critical discussions take place with family members regarding your final years and end of life planning before you are in a crisis. Jackie will provide advice on how to have those conversations, issues to be addressed and what key documents you should have in order.
